UFHealth.org Locations

UFHealth.org serves as the primary point of access point for UF Health’s patients, potential patients, their families and the community. A key component of the site is our locations directory, which allow users to research our hospitals and clinics. It is vital to have information on our locations that is accurate, informative, and up-to-date at all times.

To add a new location, please fill out the form below as completely as possible. The information provided is used to connect the location with relevant health conditions, services, providers and is essential to providing patients with complete information to make informed healthcare decisions.

Add a UF Health Location

Please note that many fields are required. Please submit the form when you have all the information necessary for the required fields.

New UFHealth.org Location Form

  • A description of services offered including any marketing material you would like on the location page.
  • A short location description (used mostly for listings management). 750 characters max.
  • Accepted file types: jpg, gif, png, Max. file size: 125 MB.
  • Hold down the CTRL key to select more than one option.
  • Please use the UF Health Encyclopedia to create a listing of up to 25 of the most important health topics to associate with the location.
